Patient Centered Medical Home (PCMH)

Naval Health Clinic Quantico follows the Primary Care Medical Home model, a team-based model based on the premise that the best healthcare begins with a strong primary care foundation, accompanied by quality and resource efficiency incentives. The hallmark of a Medical Home is enhanced continuity of care with a focus on prevention, providing continuous, accessible, family-centered, comprehensive, compassionate, and culturally-sensitive care in order to achieve the best health outcomes.

Each patient is assigned a Primary Care Manager (PCM) who is supported by a team of nursing, embedded specialty, and administrative staff who work together to manage the health of their enrolled population.

See your PCM

• When you have a non emergent medical need, your PCM is the first stop you should make.
• If your PCM has no availabilities that day, another provider on the team can see you.
• The continuity of care is more personal and better coordinated because you are known by the support staff that interact with you every time you visit your Medical Home
